Re-ablement service with trained Enablers in Elderly care, Dementia and Physical Disabilities

Re-ablement
Re-ablement defined by the Department of Health Care Services Efficiency Delivery (CSED) Programme is
“Services for People with poor Physical or Mental Health to help them accommodate their illness by learning or re-learning the skills necessary for daily living”.
Re-ablement aims to:
Assist you in restoring your confidence in undertaking everyday tasks and activities, following a period of illness, or when leaving hospital.
- Help you maintain independence in your own home rather than dependence on family or services.
- Support you in attending therapy sessions and carrying out exercise programmes prescribed by Health Professionals.
- Enable you to learn new skills to help you manage in your own home.
- Provide an opportunity for you to determine the support you need.
Who Provides the Service?
Loga Care has highly trained staff in re-ablement called enablers who work with Re-ablement professionals. Loga Care does not provide specialist advice on re-ablement.
Loga Care will assess each person and in conjunction with you and work on a re-ablement plan. This plan will be under the direction of a specialist team on re-ablement. The aim of which is for you to remain in your home or provide the opportunity for you to assess whether you can manage independently at home.
The service will be on a live in basis providing the additional reassurance of support on a 24hr basis.
Loga Care Enablers will:
- Work with you on a re-ablement care plan which will be personalised to your re-ablement needs.
- Support you in all activities of daily living to include: personal care, meal preparation, shopping, household tasks and linking with community activities .The level of care will reduce during the re-ablement period to enable you to live independently or with the support of carers.
- Implement the re-ablement plan to help you improve and develop your skills with the same Enabler to provide stability and consistency.
- Refer to specialist re-ablement staff as required, for example Occupational Therapy.
- Inform carers and family of the level of independence you have achieved.
- Work with other agencies to with regard to your ongoing care eg. District Nurse
- Monitor and record progress to include physical, psychological, social, emotional aspects.
- Assist you in identifying the level of support you need to enable you to live at home.
What happens at the end of the re-ablement period?
It is hoped that the intensive support you have received from Loga Care will result in you requiring less or no support at home, however everyone’s needs are different and you may need to discuss with Adult services the choices available to you.
Re-ablement from Loga Care offers you the opportunity to maintain independence and lifestyle choices.
